Your insurane carrier, adjustor, or attorney has scheduled an  appointment on your behalf (exception in the case of Panel Qualified Medical Evaluations or Panel QMEs, whereby the patient is directly responsible for scheduling appointments). Patients are required to appear at all scheduled appointments.

 

Appointments rescheduled with less than a two-week notice are considered as no-show appointments.

Please bring the following to your appointment:

Dress comfortably in loose clothing, perhaps in an exercise suit. Dr. Henry will examine you and ask questions to compile a complete medical history to aid in planning your treatment.
