Medical-Legal Instructions
Your insurane carrier, adjustor, or attorney has scheduled an  appointment on your behalf (exception in the case of panel Qualified Medical Evaluations, or Panel QMEs, whereby patient is directly responsible for scheduling appointment). Patients are responsible for appearing at all scheduled appointments.
Medical-Legal Patient: Preparing for Initial Appointment
Please arrive 30 minutes early to fill out the necessary forms.
 
Appointments rescheduled with less than a two-week notice are considered as no-show appointments.
 
If you are having an Electromyogram study done, please review the instructions on Preparing for an Electromyogram.
 
 
If you have had a CT scan, x-rays, or MRI study done pertaining to your case, please ask your physician for a copy of these reports to bring to your appointment. Your doctor may prefer to send or fax the reports directly to Dr. Henry's Kentfield office (see address above at right).
